Athlete Registration
ATHLETE ELIGIBILITY
To participate in Special Olympics Wisconsin, a person must meet certain criteria and agree to uphold the Special Olympics Athlete Code of Conduct.
REGISTRATION
If eligible for participation, all athletes—new and current—must complete the Athlete Registration Form and Athlete Code of Conduct.

- Health history, medications, and treatments
- Waivers and policies covering risks, use of likeness, emergency medical care, consent for health program participation, personal information, and other key details
In certain circumstances, athletes will be required to complete additional forms and will be notified if necessary.
Registration and the Athlete Code of Conduct must be completed every year.
SPORT DEADLINES
To be eligible for state competition, athletes must submit all required registration paperwork by the sport-specific paperwork deadline.
January 1
• Alpine Skiing, Cross-Country Skiing, Snowboarding, Snowshoe Racing
February 1
• 3v3 Half-Court Basketball, Basketball Skills, Basketball Team, Swimming
April 15
• Cornhole, Powerlifting, Soccer, Tennis, Track & Field
July 15
• Bocce, Flag Football, Golf, Softball & Tee Ball
October 1
• Bowling, Volleyball

Register in the Special Olympics Portal
Visit the Special Olympics Portal link below and complete all required online forms.
All athletes—new and returning—must begin here when joining or updating their information.
You must have a valid email address to use the Special Olympics Portal.
- First time users: Select Create an Account
1. Select Wisconsin as the program you participate in
2. Select your role. Athletes who are their own guardian will create their own account to register themselves. Parents/Guardians/Family members will create an account to register athletes under the age of 18 or who are not their own guardians.
